The information tab holds all your patient’s general information, such as:
- Contact info, e.g. name, mobile number, email, date of birth, gender and address.
- The linked practitioner
- Referral sources
- Custom fields
- Notes
The most important details on this page are the email and date of birth, this is what SimpleClinic uses to identify patients and make sure their bookings, forms and patient portal content are all linked to the right patient.
If you find yourself with more than one patient record, this usually means a patient has created a booking with a different email or date of birth. In this case you need to merge the patient files, and it is important to follow the knowledge base article, so you get all the information on the right record.
If a patient completes an intake form, with a different email or date of birth then it won’t link to their patient file, and you will need to find it under the Responses menu and link it yourself.
Notes can be added here for anything you need to remember.
If you need more than one notes field, you can add custom fields for any extra information you wish to keep handy. Custom fields can be set up as alerts, so you get reminded every time you access the patient file.
Actually, the most important thing on this page is the SAVE button -> make sure you always hit save after updating patient information.
