Unlinked forms happen if the email or DOB in the form submitted back isn't the same as the one they booked in with. The forms match back based on the email address and DOB as these are the most unique fields across all patients.

If a form is unlinked, first confirm this by reviewing the notification email sent to your email address when the form was sent back. At the bottom of the email, it will tell if the form was unlinked or if it linked with another client.


We also suggest check family members with the same email or archived clients if you can't find the form.


If not linked to a client at all you would have received a notification to your email inbox.


You have two places where you can access form responses:


1) Firstly from Content -> Responses -> View the form -> Click the client name and a dropdown box will appear -> Type the new client it needs to be matched with and the form will be moved straight away. Steps below:
