Unlinked forms happen if the email or DOB in the form submitted isn't the same as the email/DOB they booked in with. The forms match back based on the email address and DOB as these are the most unique fields across all patients.

If a form is unlinked, first confirm this by reviewing the notification email sent to your email address when the form was sent back. At the bottom of the email, it will tell if the form was unlinked or if it linked with another client.


We also suggest checking family members with the same email or archived clients if you can't find the form.


If not linked to a client at all you would have received a notification to your email inbox.


You have two places where you can access form responses:


1) Firstly from Content -> Select patient record -> Click the view button next to the unlinked form -> select MOVE FORM next to the Patient Name field -> Search for the correct patient name -> Chose SELECT next to the correct patient name. Steps below:
